A medical report may be required on occasions such as commencing a life assurance policy, pension, or mortgage policy. The insurance company concerned will send a written request and medical questionnaire to your doctor.
It may or may not be necessary to attend the surgery for an appointment and the insurance company will instruct you in this regard. Please advise reception that you are attending at the request of your insurance company for a medical examination at the time of booking an appointment, since a longer appointment is usually necessary.
